Share information on new developments, technology, or
patient care issues related to
perioperative practice in the AORN Journal. Articles on Clinical,
management and education topics are needed to provide timely
information. The journal is looking for manuscripts that report research related to preoperative, intraoperative or immediate postoperative
patient care. Of particular interest is research that reports surgical patient outcomes.
Manuscripts should be longer than 12 pages and include a 100 word abstract. Submit three copies of the manuscript, an outline and a cover letter that includes the home, business, and e-mail
addresses, telephone and fax
numbers of all authors. Author guidelines are available online, or by

uestion: At our facility, before a patient arrives in the OR, an anesthesia care provider typically warms the OR bed by placing the warming unit hose under the sheets. The warming unit is turned off when the patient is moved to the OR bed. After the patient is induced and his or her skin is prepped, the warming unit and hose are turned on again. Many perioperative nurses are concerned about this practice because blowing air from the warming unit hose stirs up bacteria and increases the risk of infection. What is AORN’s recommendations?A
nswer: Using the hose to blow warm air directly under the sheets without the designated warming blanket is not recommended. This inappropriate use of a forced air warming system often is called “hosing.”Misuse of forced air warming systems creates an increased risk of infection for patients. Air blowing freely from the hose increases air currents and the amount of airborne particulate matter. These airborne particles comprise dust, lint, skin squames, and respiratory droplets that may contain microbacteria. Increased air currents can carry these contaminated particles to the sterile field.
Strains of bacteria traced to outbreaks of surgical site
infections have been recovered from air in ORs.
The air temperature at the end of the hose is elevated, so using the hose without the warming blanket also places patients at risk for thermal injuries. Extended exposure to concentrated heated air that is flowing freely under the sheets increases the risk of injury. The risk of injury from the warming hose is greater for pediatric patients and patients with vascular compromise.
Maintaining patients’ temperature during all phases of surgical intervention is fundamentally important. When indicated, forced air warming systems provide an effective method of maintaining patients temperatures. Forced air warming systems are intended to be used with specifically designed, sealed blankets.
The manufacturer’s instruction should be followed.
